Treatment of the Treatoma in Pediatric Surgical Patients

Abstract
The teratoma is a unique complex neoplasm, it is one of the most frequently pediatric tumor in extragonadal germ cell tumors. Sacrococcygeal and retroperitoneal teratomas were considered to be rare congenital anomalies.
Mature teratomas are entirely composed of mature differentiated tissues, while immature teratomas always contain in addition embryonic tissues of variable degree of immaturity, especially neuropeithelial elements. Diagnosis of teratoma is
relatively
easy by conventional radiologic study, but the discrimination between mature and immature teratoma before confirming by histopathlogical diagnosis is difficult.
To determine the extent of the treatment of teratomas, 8 consecutive cases of sacrococcygeal(4) and retroperitoneal(4) teratomas from January 1989 to February 1993, were studied retrospectively at Department of Surgery in Chonnam University
Hospital.
@ES Results were as follows;
@EN 1) Female to male ratio was 3 : 1, and age distribution was most prevalent below first year of age(62.5%).
2) Among 8 cases of teratoma, sacrococcygeal and retroperitoneal types were each 4 cases. Ad the major symptom and physical finding were mass palpation.
3) The operative mode was complete excision of mass, and no major complication was noted,
4) All 4 sacrococcygeal teratomas and one of 4 retroperitoneal teratomas were mature teratomas. And there was no evidence of recurrence after complete excision.
5) Three retroperitoneal teratomas were immature teratomas, 1 case of grade I, 2 cases of grade¥².
6) The preoperative alpha-fetoprotein level of all immature teratomas was higher than normal level according to Tsuchida graph, and progressively declined to normal range at 1 month postoperatively in 2 cases.
7) 3 immature teratomas were combined with multi-agent PEB chemotherapy(Cisplatin 40 mg/m2, Etoposide 100 mg/m2) and 2 cases were well managed to the aggressive chemotherapy and follow-up with good condition, but 1 case(grade¥²) was expired from
severe
bone marrow depression due to chemotoxicity.
